The Elizabeth Hospice Pediatric RN Case Manager is
Work Schedule: 8:00 a.m.-5:00 p.m. Monday -Friday
On Call: 2 shifts per month-typically 1 weekday and 1 weekend shift.
Salary: $40-$50 Hourly. Subject to experience and qualifications.
Regular Full-Time
Travel required for patient visits.
Occasional travel for educational purposes.
Weekend commitment: on call only.
The Elizabeth Hospice Center for Compassionate Care Programs Manager is responsible for the development and oversight of The Elizabeth Hospice (TEH) Grief Services Center for Compassionate Care. Supervises the Bereavement Coordinator to ensure that the assessment, treatment planning, support and counseling needs of TEH patients and their families are met as well as working with the counseling staff to meet the needs of the community bereaved. Assesses, counsels, treats, and educates TEH survivors and community members, including those considered at high risk and with complicated grief. Maintains accurate documentation of sessions, interventions, plans of care, etc. Provides clinical supervision to student counselors and staff pursuing licensure. Responsible for identifying and developing educational programs for families and staff on loss, grief, and related topics. Oversees bereavement follow-up by members of the patient care staff and volunteers.
Work Schedule: Monday-Friday 8:00 a.m.-5:00 p.m.
This is a salaried, exempt, full-time position.
Annual Pay: $85,000-$108,000 (subject to experience and qualifiications)

HR areas of focus include: compensation, benefits, recruitment, employee relations, learning and development, leaves of absence, workers compensation, job accommodations and health and safety. Recommends new approaches, policies, and procedures to effect continual improvements in efficiency of department and services performed.

The Elizabeth Hospice Children's Bereavement Coordinator (MSW highly preferred) is responsible for planning and providing grief and loss services and education for children and teens. The Children’s Bereavement Program includes Peer – Based Support Groups, School-Based Grief Groups, Camp Spero, educational presentations to the community on topics related to children’s grief and community crisis response. This position requires local travel to offices, schools, and community organizations and requires flexible hours during both evenings and daytimes and occasional weekends.
Work Schedule: Monday-Friday 8 hour shift (Full-time salaried position).
Salary: $65,000-$80,000 Subject to experience and qualifications.

The Hospital Liaison RN at The Elizabeth Hospice, initiates, establishes, develops and maintains professional relationships between hospice and various referral sources, e.g., hospitals, skilled nursing, assisted living facilities, RCFEs, etc., physician associations and independent medical clinics/offices in order to assure a consistent base and flow of patient referrals across the life span (perinatal, pediatrics, and adults). Takes a pro-active role in helping to grow the census. Effectively collaborates with admission team to assist in implementation and coordination of services for discharge of patients from hospitals into TEH’s program of care. Educates the medical community about the benefits of hospice services. Displays an ongoing commitment to excellence in customer service to both internal and external customers.

The Elizabeth Hospice MSW (Medical Social Worker) is a member of the Interdisciplinary Group (IDG), the MSW assesses the psychosocial impact of the terminal illness on the individual and their family/caregiver system. Intervenes to support, guide and prepare them as they cope with the disease progression, death and grief. The MSW primarily conducts visits in patients’ homes, facilities, and in the community and coordinates care with family members, caregivers and the IDG.
Work Schedule: Part-Time Saturdays and Sundays 8 hour shifts
Salary: $26,000-$32,000 (Based on a 16 hour work week. Salary determination based on MSW and hospice experience).
Part-Time Salaried Position
Travel required for patient visits.
Occasional travel for educational purposes.
Weekend commitment: Every weekend

The Elizabeth Hospice Respiratory Therapist assesses, evaluates and develops a treatment plan and provides quality therapy treatments that meet each patient’s individual needs and that are medically prescribed and necessary using the standards of professional practice. Exercises independent judgment for clinical decision making and prepares all documentation required to support all aspects of patient care provided. Participates as a member of the interdisciplinary team and the plan of care for patients across the life span (perinatal, pediatrics, and adults). Performs other duties as assigned. Follows TEH rules, policies, procedures, applicable laws, and standards. Carries out the mission, vision, values, and quality commitment of TEH.

The Elizabeth Hospice Chief Medical Officer is the Medical Director with overall responsibility for the medical component of the hospice and palliative care programs. The Medical Director will provide oversight of physician services by acting as a medical resource to the interdisciplinary group, assuring continuity of hospice medical services, and assuring appropriate measures to control patient symptoms. The Medical Director will serve as a hospice and palliative care champion – promoting and representing the program to physicians, physician groups, discharge planners, other referral sources, community health organizations, and potential donors, as appropriate.
Work Schedule: M-F, 8:30am-5:00pm
Salary: $233,000.00-$325,000.00. Subject to experience and qualifications.

Degree of Travel: : Moderate for program supervision, public relations and marketing. Attendance at various seminars, conferences and workshops may be required from time to time
