The Elizabeth Hospice RN Case Manager is primarily responsible for providing professional nursing care to assigned patients, which includes adults, pediatric and perinatal. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice.   Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. On Call: 2 shifts monthly (may require weekend call) Hourly Pay: $45.00-$65.00 Hourly. Would consider Part-Time/Per Diem Work hours: NOC Shift 9:30pm-8:30am Weekend Commitment: 2 Shifts On Call Frequency: 1 Shift Per Month

Flexible to be trained in Admissions.   Work Schedule:   Various days, 4:00 p.m. - 12:30 a.m. Salary: $35.00 - $45.00 Subject to experience and qualifications. Per Diem Position - candidate must provide 6 shifts, 2 of which are weekend days, per month.  Shift availability does not guarantee hours. Travel required for patient visits Occasional travel for educational purposes. Subject to experience and qualifications. All noc shift RNs will receive additional differential pay when working 7:00pm-7:00am     Travel required for patient visits. Occasional travel for educational purposes. Weekend commitment:  Every-other-weekend.
