The Patient Care Coordinator (PCC) LVN is primarily responsible for patients’ clerical and communication care coordination from admission to discharge. Performs a wide variety of clerical and if applicable clinical duties for the Pediatric and Palliative Care programs assigned to including, but not limited to, processing and maintaining patients’ clinical records, Interdisciplinary Group (IDG) prep, processing and tracking of physicians’ orders, processing patient authorization requests, certifications, and ensuring seamless communication between patients, staff, and team manager. Serves as key communication conduit for Pediatric and Palliative Care programs, the Medical Group within the agency and for outside communication.

The Elizabeth Hospice MSW (Medical Social Worker) is a member of the Interdisciplinary Group (IDG), the MSW assesses the psychosocial impact of the terminal illness on the individual and their family/caregiver system. Intervenes to support, guide and prepare them as they cope with the disease progression, death and grief. The MSW primarily conducts visits in patients’ homes, facilities, and in the community and coordinates care with family members, caregivers and the IDG.
Work Schedule: Monday-Friday 8:00 a.m.-5:00 p.m.
Salary: $70,000-$80,000 annually
