Overview

Primarily responsible for providing professional nursing care to assigned patients. Initiates, establishes, develops and maintains professional relationships between the Palliative Care Program and various referral sources, e.g., hospitals, skilled nursing, assisted living facilities, RCFEs, etc., physician associations and independent medical clinics/offices in order to assure a consistent base and flow of patient referrals across the life span.  Takes a pro-active role in helping to grow the Palliative Care census. Effectively collaborates with admission team to assist in implementation and  Educates the medical community about the benefits of palliative care services. Displays an ongoing commitment to excellence in customer service to both internal and external customers.

Work Schedule: Monday through Friday, 8:30 a.m.-5:00 p.m. 

Salary: $45.00-$61.00/hr Subject to experience and qualifications including years of licensure

Work Location: Escondido, CA

Travel: Required for patient visits

Responsibilities

  1. Proactively facilitates the palliative care referral process while prioritizing patient needs. Maintains good public relations with community referral sources, managing time effectively
  2. Effectively collaborates with the hospital based TEH palliative care physician to assist in implementation and coordination of services for discharge of patients from hospitals into TEH’s program of care, maintains a strong customer service approach.
  3. Collaboratively works with the Admissions and the palliative care department to troubleshoot real or perceived problems identified by referral sources and resolve customer complaints.
  4. Completes admission of patients to TEH palliative care program in coordination with the referral source patient and family. Completes community based palliative care field admissions as needed and situation indicates.
  5. Develops written plan of care in coordination with an Interdisciplinary Team (IDT).
  6. Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs.
  7. Demonstrates competencies with excellent nursing skills and care planning while providing direct patient care.
  8. Assesses patient/family need for psychosocial support and implement services (i.e. Chaplain, MSW, volunteer) as indicated
  9. Works effectively and collaboratively with the medical and clinical staff and community physicians by evaluating and assisting patient/families with the appropriate plan of care.
  10. Establishes, builds and nurtures relationships with various identified sources in the local and regional medical community including community hospitals. Participates in in-service and marketing activities as needed.
  11. Maintains timely patient documentation using “point of care” electronic charting.
  12. Provides goals of care information to patient and family; documents for the team the wishes of the patient and assists with the completion of POLST, DNR as requested by family.
  13. Introduces the concept of hospice at the appropriate time and refer to hospice if needed/desired by the patient/family. May complete hospice admissions.
  14. Assesses patient/family need for psychosocial support and implement services as needed
  15. Communicates all changes to IDT and community physician.
  16. Manages end of life symptoms proactively.
  17. Participates in training and mentoring of new staff.
  18. Keeps current with all department policies and procedures.
  19. Adhering to the E-way standards is a requirement in performing all job duties.
  20. Other job duties as assigned.

Qualifications

  1. Bachelor’s Degree in nursing; certification as hospice and palliative care nurse preferred.   One year in acute care setting or equivalent combination of education and experience required. 1-2 Years Hospice/palliative care experience preferred.
  2. Current CA RN professional license.
  3. Demonstrated ability to work effectively with patients, physicians, community agencies and institutions. Interest and expertise in marketing preferred.
  4. Demonstrated ability to communicate effectively with members of the hospice interdisciplinary team.
  5. Current CPR certification and negative TB skin test.
  6. Proficient computer skills required.
  7. Demonstrated ability to multi-task, and manage crisis situations.
  8. Current driver’s license with proof of auto insurance.
  9. Bi-lingual (Spanish/English) preferred.
