Overview

The Elizabeth Hospice, Hospice Aide is primarily responsible for providing direct personal care to terminal patients in their homes. Also responsible for participating in patient team updates. Responsible for carrying out the mission, vision, values, and quality commitment of The Elizabeth Hospice.

 

The Elizabeth Hospice is San Diego’s oldest non-profit hospice of 45 years.  We are growing and looking for talented individuals to join our high performing team and support our mission.  We serve all of San Diego county and south Riverside county with multiple hospice office locations.  

 

Our Mission:  To enhance the quality of life for those nearing the end of life’s journey and for those who grieve.

 

Hourly Pay: $22.00-$26.00 Hourly. Subject to experience and qualifications.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m.

Work Location: San Diego, CA (Miramar north and east including Scripps Ranch, Poway, Ramona, Mesa Grande, Julian, and Banner)

Travel Required for patient visits. 

Responsibilities

  1. Provides personal care to patients in their homes, SNFs, & RCFEs with consideration for age-related differences.  Meets their Activities of Daily Living needs as assigned by RN.
  2. Coordinates personal care services, scheduling visits with patient/family.
  3. Observes and communicates changes in the patient’s condition to the primary RN and/or Team Manager and staff at facilities as needed.
  4. Attends discipline-specific and all-staff meetings and maintains continuing education credits for licensure.
  5. Recognizes needs of patients/families and responds appropriately within the scope of practice.
  6. Performs related tasks per plan of care which may include staffing crisis care, making deliveries, light housekeeping, and other tasks as assigned.
  7. Documents observations accurately according to charting standards.
  8. Maintains timely patient documentation using “point of care” electronic charting.
  9. Performs basic skills included in competencies checklist, policies/procedures and standards that are required for agency.
  10. Reports to primary RN or Team Manager any changes of condition, injuries of unknown origin, or  patient/family/facility concerns in a timely manner.
  11. Adhering to the E-way standards is a requirement in performing all job duties.

Qualifications

  1. Post high school Vocational/Specialized Training.
  2. Minimum of 6 months experience (waived with special consideration of Team Manager.)
  3. Preferred experience of 1-2 years direct patient care.
  4. Must possess current California CHHA and CNA Certification.
  5. Current CA driver’s license, proof of insurance, and reliable transportation.
  6. Spanish speaking preferred.
  7. Computer skills (word processing), use of calculator, fax, and photocopier.
  8. Because this is a field position, must be able to drive in various conditions, including but not limited to windy/steep roads, night driving, and rural areas, and be able to enter homes with pets.
