The Patient Care Coordinator (PCC) LVN is primarily responsible for patients’ clerical and communication care coordination from admission to discharge. Performs a wide variety of clerical and if applicable clinical duties for the Pediatric and Palliative Care programs assigned to including, but not limited to, processing and maintaining patients’ clinical records, Interdisciplinary Group (IDG) prep, processing and tracking of physicians’ orders, processing patient authorization requests, certifications, and ensuring seamless communication between patients, staff, and team manager. Serves as key communication conduit for Pediatric and Palliative Care programs, the Medical Group within the agency and for outside communication.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m., on-call availability preferred. (On-call schedule: 1 week day NOC shift from 5pm - 8am per week and 1 weekend day DAY or EVENING shift per month.)
Salary: $32 - $36 hourly. Subject to experience and qualifications.
Work Location: Escondido, CA
Travel:Travel throughout San Diego County may required for patient visits, training, or office visits.
